AAFO SCHOLARSHIP APPLICATION PROCEDURES

1. Applicant is responsible for submitting a complete packet of information that includes
meeting conditions described, and all items from check-off list. Incomplete packets will
not be considered for review.

2. This form should be signed and submitted with entire, completed AAFO Scholarship
application packet via U.S. Mail to the: Asian American foundation of Oregon, P. O. Box
51117, Eugene, OR 97405, with a postmark no later than April 15 of each application year.
Submissions arriving after this deadline will not be considered eligible.

3. All eligible applications will be reviewed by an appointed AAFO Scholarship & Selection
Committee. Recipients will be selected and notified by May 1st of each application year.
Scholarship recipients will be invited to attend the Annual Meeting of the AAFO to receive
their awards.

5. AAFO Scholarship grants are open to any person who is a resident of Oregon,
who has demonstrated interest and effort in Asian American cultural activities such as:
participation and/or volunteer work in their community's Asian community or cultural events, or for any AAFO sponsored activities such as the annual Asian
Celebration, Kite Festival, Asian Arts Auction or Obon. AAFO Scholarships are open to high
school students accepted for admission to colleges and college undergraduate students only.
Graduate students and students pursuing second degrees are not eligible.

6. Applicants of Asian American heritage are strongly urged to apply.

7. Applicants must submit an Outline Form Chronological Summary of Participation in Asian
American cultural activities, including: year(s), name of activity, place of activity,
description of role(s, number of hours and the name/phone number of event contact.

8. Enclose at least one (1) letter of recommendation describing interest and commitment to
Asian American community and culture, demonstrated by applicant. Recommendation letter must
be from a person not related to applicant and having personal knowledge of applicant's activities.

9. Applicants may be members of, or related to members of the AAFO, with the exception of those
who are members of, or related to the decision makers on the Scholarship/Selection Committee.

10. Applicants must have a minimum current grade point average (GPA) of 2.5, or if lower, must
submit documentary evidence of acceptance by a college.

11. Applicants must submit an essay of 250 words maximum that describes their college plans,
career interests, relationship to and interest in Asian American culture and/or future commitment
to Asian American community.

12. Applicant must provide the AAFO with a written progress report and ending grade transcript
for school year, no later than April 30 of award year.

13. All AAFO Awards must be used for Higher Education expenses for the academic year awarded.
Award is non-transferable and must be used by the recipient. In the event that grant is not used
during the year awarded, recipient must return all funds to the AAFO by June 30 of the same
academic year.

10. Applicants are allowed to re-apply in future years.
